It's a matter of arching the lower back, which throws the spine out of proper alignment leading to lumbar problems. It also throws all the weight onto the ball of the foot leading to stress on the metatarsals and phalanges, hairline fractures, inflammation and things like plantar fasciitis. If the shoe is also pointed, it tends to cause the toes to eventually slide into the narrowest part of the shoe, leading to deformations of the joints, bursitis, arthritis and bunions. Additionally, wearing such shoes on a continual basis causes shortening of the Achilles tendon, making it difficult to walk at all in flat shoes.
