Had he not caused the Shah to moderate the Savak, there would have been no Islamic Revolution. History would be completely different - all Shiite terror since 1979 would not have occurred. Israel would not have had to deal with Hezbollah. Lebanon would not have collapsed.
Not sure what, if any affect this alternative history would have on extreme Sunni movements.
But Carter was the sovereign of a super power. He wasn't the recognized leader of a back-woods commune. His actions directly caused Iran to fall to Shiite extremism.

The Savak handled the dissidents very well. Carter forced the Shah to rein the Savak in. The revolution followed....
