But they cannot know the purity as it is illegal. If that illegality limited access it might be worth it. But it doesn't. Imagine drinking all night and having no way of knowing if you were ingesting weak beer or grain alcohol until someone else found you the next day.

But they start with the pill. Correct me if I am wrong, but in USA is very common to give an opioid for a headache. While in Spain, it would be the very last resource, and even in those cases it would be avoided.

Every opioid even in pill form is extremely addictive, and even starting with this, then you get addicted and you need stronger dosis, and it is when you go for heroine. And then you are finally screwed.

The US is trying to move away from opiods. For people who have taken them for long periods of time- they do tend to get addicted, but pain management doctors call it a dependence rather than an addiction.
