Approximately 23.5 million people have autoimmune disorders in the US as of 2018. Of that, approximately 25% have multiple autoimmune disorders (MAS), so just shy of 6 million people have MAS. The same situation that I deal with, would apply to that 6 million, and that's not inclusive of others with underlying conditions not considered to be autoimmune. The need to make a choice about an injection that could very likely cause an imbalance in the autoimmune situation for that individual. It could be anywhere from virtually no issue, to catastrophic. Since there was no data to compare the impact on people dealing MAS, not taking it was the preferred response.
If you want to call that fear, then I guess you will. I call it understanding once's body, personal health issues, and following the path to avoid a negative response.
But, as with most words grossly overused, they lose their value.

We (my family) did a lot of research. Based on the people it was mostly affecting, and our health status, we chose not to. A couple people were being kind of forced, (college students, & health care workers) with everything we knew taken into account, it was determined if it had to be taken to take the J&J. No one took a booster, but were able to show they were vaccinated.
